Providing quality health care proxy services when patients lack representation and the ability to make their own decisions. Social Work Advantage health proxy services assist health care facilities with these difficult important patient decisions.

With our new office in Sarasota, we serve the West coast of Florida and Central Florida.
We are Social Work Advantage. We contract with health care facilites to provide health proxy services for patients unable to make health care decisions and have no one else to assist them in decision making.
Florida Statue 765.401: a licensed clinical social worker, pursuant to chapter 491, may serve as the health care proxy for a patient unable to make decisions for himself or herself and when there is no other legally authorized representative.
Social Work Advantage Health Proxy Services may be contacted to make health care decisions, in the best interest of the patient, which may include decisions to withdraw / withhold life-prolonging procedures. Decisions to withhold or withdraw life-prolonging procedures will be reviewed by the facility's bioethics committee.
When it is discovered by the contracted hospital or hospice that the patient:
- Lacks capacity to make health care decisions
- Has no one legally authorized to assist in making health care decisions
- There has been documentation of the above in the medical recored.
