Health Proxy Services
Carol, owner of Social Work Advantage, has extensive clinical experience in medical social work and bioethical patient care. She has served as the chair of a large hospital ethics committee, as well as the director of a Hospice program and developer of a Palliative Care team. She is committed to provide appropriate care through this proxy service to those who need to be represented.
Alison Moore, LCSW joined the central team for Social Work Advantage and has an extensive background in working with patients in a medical setting. She has worked in hospital and home health settings as well as a private practice therapist. She graduated from Florida State University and the University of South Florida. She has a special interest in providing ethical decision making as she works with patients who have no one to advocate for them.
Caroline Evans Guida, LCSW has joined SWA after having many years of experience as the Director of Social Work in a Ft. Lauderdale Hospital and later, as a contract administrator. She comes with a background in administration and medical social work as well as Psychotherapy. She has her MSW from Florida State University and is part of the Academy of Certified Social Workers.
Luisa Betancourt, LCSW has a background in medical social work and hospice. She has experience and skills in the psychosocial care of dying individuals, bereaved families, and groups dealing with complicated grieving.Ê Luisa has had extensive exposure to different cultures and communities through years of volunteer work in Latin America, Europe, and South Florida. She has her MSW from Barry University and is part of the Academy of Certified Social Worker and Advance Certified Hospice and Palliative Care Social Worker.
Only Social Workers who have clinical medical experience are part of Social Work Advantage. All are committed to making ethical and appropriate decisions for their patients and the facilities that care for them.
Bernadette Brunetti, LCSW, has almost 20 years experience working as a medical social worker in outpatient and acute care. She has worked in several settings including a large public hospital system. Her experience includes working with trauma, transplant, and hematology/oncology patients and their families. She also has experience in end of life care and has been part of a hospital bioethics team. She brings a unique perspective to the SWA health proxy team.
Sandra Richards, LCSW is part of the central team of Social Work Advantage. She has a background of medical social work and behavioral health. She has an interest in bioethics and has served as a director of a social service team. She has her MSW from FSU and works with her other team members from the Sarasota Office. She has a special interest in working with patients who are in the medical setting.
Jennifer Pejoves is a LCSW who has experience in working in the Guardian ad Litem Program for Miami as well as working in a Palliative care program and a Hospice Service. She is fluent in English and Spanish and has a professional experience working with diverse cultural populations. She has a passion for social work and is dedicated to providing quality and ethical decision making for all of her health care proxy patients.
Andrea Santiago Malcolm, LCSW has a background in medical social work and public works. She has her MSW from Florida State University and has a special interest in Knitting as a form of therapy. She comes with an understanding of bioethics and working as a team member in a medical setting. She enjoys the opportunity of making a difference in the lives of others on a daily basis.
Yasmine Holbrook, LCSW is a social worker who has been part of NASW Florida board of directors. She has served as a social worker in private practice and in a medical setting. Her background allows her to participate in dialog with Creole speaking patients. Her education was completed in South Florida at Florida Atlantic University, where she received her MSW. She has a special understanding of dementia patients and has earned her CARES Florida Level 1 certification.
Kathryn Dumont, LCSW has a background in medical social work and has worked in various systems including acute care, hospice and mental health. She has developed and served on ethics committees and written policies and processes related to ethics and advance directives. Her experience in intensive care, medical trauma and end of life care lends both the expertise and compassion necessary to serve in the role as a health care proxy.
Barbara Bodeep, LCSW works with the central team of Social Work Advantage. She and her colleagues to the north work with contracted facilities in the Central and West Coast of Florida. She has a long background in hospice social work and medical social work. She earned her MSW from Rutgers University and enjoys working with the medical team to make appropriate decisions for patients.
